Boab Health Services is committed to supporting people of the Kimberley to access primary care service by supporting people to self-manage their chronic health conditions.  We do this by taking a holistic approach to care coordination with a focus on health literacy, health education and advocacy.

ITC Program

Our Integrated Team Care (ITC) program provides a care coordination and supplementary service program for Aboriginal and Torres Strait Islander people with chronic health conditions.

Health Navigator role

Our Health Navigator works within our Allied Health team to help people living with a chronic health condition to navigate the local health system, access local supports, learn new skills, and set goals to self-manage their chronic health condition/s.
