In partnership with numerous health and other sector agencies in the Kimberley, Boab Health Services has a long-standing role in providing quality primary healthcare Allied Health and Mental Health services in the region as well as an Integrated Team Care program.
Our clinical teams use MMEx as our web-based e-Health platform built on the foundation of comprehensive shared care, supporting a person-centered approach to clinical decision-making and outcomes.

Boab Health offers a range of programs to meet the health and wellbeing needs of our community. These programs and services include:
Integrated Team Care (ITC) program
Referral Forms
We require referral forms completed by GPs for most of our programs.
- Mental health referral form[AP1] (must be completed by a GP)
- Mental health K10 Scale
- Allied health referral form – Diabetes Educators, Podiatrists, Dietitians and Paediatric Dietitians (must be completed by a GP or other health professional)
- Exercise Physiology referral form
- Integrated Team Care program referral form (must be completed by a GP)
