Chronic Conditions Care Coordinator

Supports adults who have complex chronic condition(s) to better understand and self-manage their care.

How does this service help you?

The support is tailored to individual self-management needs which may include (but not limited to) the following:

  • Health education. 
  • Support to set and work towards SMART goals.
  • Liaison with GP/ primary care provider.
  • Referral to services as required such as allied health, ITC and community services.
  • Involving family/ carer(s) as appropriate.

Who can access this service?

  • Aged 18 years and over.
  • Living in Broome.
  • Diagnosed with any of the following targeted conditions:
      • Diabetes
      • Cardiovascular disease
      • Respiratory disease
      • Morbid obesity
  • A current GPMP is preferred however not essential.

Other chronic conditions may be considered on a case-by-case basis. 

How do I access this service?

To make a referral please complete the allied health referral form which is valid for 12 months.
