How does this service help you?
The support is tailored to individual self-management needs which may include (but not limited to) the following:
- Health education.
- Support to set and work towards SMART goals.
- Liaison with GP/ primary care provider.
- Referral to services as required such as allied health, ITC and community services.
- Involving family/ carer(s) as appropriate.
Who can access this service?
- Aged 18 years and over.
- Living in Broome.
- Diagnosed with any of the following targeted conditions:

- Diabetes
- Cardiovascular disease
- Respiratory disease
- Morbid obesity

- A current GPMP is preferred however not essential.
Other chronic conditions may be considered on a case-by-case basis.
How do I access this service?
To make a referral please complete the allied health referral form which is valid for 12 months.
