Referral Forms
Jump straight to a referral form
Every Boab referral in one list. Fill the webform on screen, or download the template that drops into your practice software.
Allied Health (ICDC) Referral Form
Care coordination for clients living with chronic conditions. The webform checks required fields as you type and hands you a completed PDF to send; the templates import once into your practice software and pre-fill client details from the record.
Mental Health Referral Form
One form for all four mental health service types. Attach the K10, plus other assessment tools where completed.
Allied Health Referral Form (East Kimberley)
Dietetics, diabetes education and podiatry on one form. Tick the services you are referring to and attach a current GPCCMP where one exists.
Integrated Team Care (ITC) Referral Form
For Aboriginal and Torres Strait Islander clients living with chronic conditions. Send with the client’s GPCCMP.

Send the form and attachments through whichever channel your practice already uses. Our intake team confirms receipt and follows up with you directly.

Common questions
Frequently asked questions
Podiatry, ICDC and ITC referrals need a current GP Chronic Condition Management Plan. For dietetics and diabetes education, attach one if it exists; the referral can proceed without it. Mental health referrals use their own assessment attachments instead.
The GPCCMP replaced GP Management Plans and Team Care Arrangements from 1 July 2025. Existing plans made under the old arrangements remain valid during the transition, so send what the client has and note it on the form. If you are unsure, ring intake and ask.
No. The Medicare cap of five rebated allied health services per calendar year applies to services billed under chronic disease management. Boab Health’s funded services sit outside that cap, so referring here does not spend those sessions.
Allied health referrals are triaged into the four priority bands above, from 1 to 14 days through to 120 days. For mental health and ITC referrals, our intake team follows up with you directly after the referral arrives. Ring us if you need to know where a referral is up to.
For mental health, yes: Low Intensity Therapy accepts self referral, and any referrer can send the form with the client’s consent. Allied health needs the referral form, which any health professional can complete. ITC is GP referral only.
Send the referral anyway with a note, or ring first. If a service is not the right fit we say so quickly and point you to the door that is, rather than letting the referral sit.
