Through partnerships with healthcare providers and community organisations, we offer access to primary healthcare, allied health, mental health services, and Integrated Team Care.

Mental Health & Wellbeing
Support when life feels heavy, by planned appointment.
- Feeling low, worried, stressed or not yourself lately
- Aged 16 or over
- Anyone can send the referral: you, family, a school or another service

Care coordination
Integrated Chronic Disease Care
A team around you when a chronic condition takes managing.
- Aged 18 or over
- Living with, or at high risk of, diabetes, a heart condition, a lung condition or a BMI of 40 or more

Dietetics
Practical food support that fits your life and your country.
- Living with a long-term health condition, such as diabetes, heart or kidney problems
- Or wanting support to prevent one

Understand your diabetes and manage it your way.

High risk foot care
Podiatry
Foot care for people whose health condition puts their feet at risk.
- Living with diabetes, kidney disease or Hansen's disease
- And there is a foot concern, such as a wound, an ulcer or numbness

- For Aboriginal and Torres Strait Islander people living with long-term health conditions
- Practical help with appointments, travel and medical aids

Many people match two or three cards. That is how the programs are designed, and our intake team confirms eligibility for every referral, so a maybe is all you need.
