Working Together for Better Outcomes

Boab Health’s Closing the Gap team work tirelessly to improve access for Aboriginal people with chronic disease to better access primary health care services. For example within Boab Health, the Closing the Gap team work closely with our podiatrists to assist clients to attend appointments and facilitate access to funding for shoes through the supplementary services program.

The photo shows Therese Howard at a recent appointment with podiatrist Amy Freeman, receiving shoes funded through the Closing the Gap Program. Transport to the appointment was provided by David Phillips, Aboriginal Outreach Worker and care coordination was provided by Dominique Shiosaki and Desarae Clements from the Closing the Gap team . The team were also assisted by Sam Cassidy, a final year podiatry student from the University of Newcastle who spent 4 weeks on placement at Boab Health with the podiatry team.
