FREE Chronic Disease Self-Management Support Program Course – Kununurra

The Chronic Disease Support Program facilitated by The Benchmarque Group is designed for health professionals, and others working within Aboriginal and/or Torres Strait Islander Health, who are new to the field of chronic disease self-management.

This is a fantastic FREE professional development opportunity for Kimberley health care providers.

The program explores a range of topics including assessing risk factors, readiness for change in health behaviours while working with both the client and health professionals to develop a chronic disease self-management plan.

This nationally accredited program is driven by a desire to create better health for clients, reduce hospital admissions, better patient compliance and the provision of care by those best qualified to provide advice. On successful completion of the programs, students should have the ability to support practitioners to work collaboratively with people managing
chronic disease.

The fully-funded (free) one-day workshop session involves several interactive group activities covering various aspects of chronic disease self-management, developing a chronic disease self-management plan and group case studies.
